To receive copies of the FOOD DIARY and HEALTH INFORMATION SHEET, simply click now on the word email and they will be sent to you (be sure to request either ADULT or CHILD forms). You can print them out, fill them in, and bring them along with you to your session.
*I also ask that you please bring with you any actual bottles of supplements that you might be taking.
For phone sessions, you will soon be able to access, fill-in, and return the forms right here! Have supplement bottles accessible.

If, however, you wanted to begin keeping a food diary right now (a minimum of 4 days is recommended), you can use the following as a guideline to help you.

A Food Diary provides critical information. If possible, do not record days in direct succession. Please, it is important that you record EVERYTHING you ingest during a TYPICAL day.
This includes gum, mints, medication, toothpaste, beverages - ANYTHING that passes your lips!

DAY 1- Day 4
Include a column for:
1. Item(s)
2. Amount
3. Time
4. Place
5. Hunger Level(1-5)

*Please list any medications and all lotions, creams, or products applied topically (deodorants, shampoo, perfume).

*Try to pay attention to how you feel after eating- tired, alert, foggy, bloated, stiff, abdominal or joint pains, alert, energized, etc. This will become much easier after modifications are made to your current diet because HOW various foods affect your body is will become far more evident. Beginning to cultivate this level of cognizance, however, is important.

For children, watch for changes that manifest physically (distention, redness around nose, ears), behaviorally (eye contact, aggressiveness, insensitivity to pain), and/or in their mood (spacey, sullen, confused).

Leaving yourself an area for "Notes" is usually helpful.

Again, THANK YOU for viewing my site and I look forward to an opportunity to work with you in achieving your health and wellness goals!

Healthy Regards!
Gina
